3. Consent to Record

I, the undersigned parent or legal guardian, hereby grant permission to Girls Who Vibe (a program of The Practice Center) to photograph, video record, and/or audio record the above-named participant during the specified event(s), including but not limited to:

Project pitches and presentations

Demo Day showcases and walkthroughs

Working sessions and collaborative building time

Interviews, testimonials, and reflections

Screen recordings of participant's project demos

Group photos and event documentation

4. Permitted Uses

I grant Girls Who Vibe the right to use, reproduce, edit, and distribute recorded media of the participant for the following purposes:

Girls Who Vibe website and social media channels

Promotional and marketing materials (digital and print)

Educational and curriculum-related content

Grant applications, reports, and fundraising materials

Press, news, and media coverage

Partner and sponsor communications

Internal training and program development

Note: Footage will never be sold to third parties. Participant's last name will not be publicly associated with media without additional written consent.

5. Opt-Out Options

I would like to exclude the following uses (check any that apply):

Do NOT use participant's image on social media

Do NOT use participant's image in press/news coverage

Do NOT use participant's full name alongside media

Do NOT share footage with external partners or sponsors

Other restrictions (describe below):

6. Terms & Understanding

By signing below, I acknowledge and agree that:

This consent is voluntary. Participation in the program is not contingent on signing this form, though the participant may not appear in recorded materials without it.
I may revoke this consent at any time by contacting info@aidedeq.org in writing. Revocation will apply to future use only; media already published or distributed prior to revocation may not be retrievable.
No compensation is provided for the use of recorded media.
Girls Who Vibe will make reasonable efforts to use media in a manner that is respectful, empowering, and aligned with the program's values.
I release Girls Who Vibe, The Practice Center, and their staff, volunteers, and partners from any claims arising from the authorized use of recorded media as described above.
